Albendazole, a benzimidazole derivative, works by interfering with the polymerisation of tubulin in the parasite. This disrupts the worm's cytoplasmic microtubules, leading to a failure in glucose uptake and the depletion of glycogen stores. Without an energy source, the parasite eventually dies. Ivermectin, an avermectin derivative, targets the glutamate-gated chloride channels in the nerve and muscle cells of invertebrates. By increasing the flow of chloride ions, it causes hyperpolarisation and immediate paralysis of the parasite, allowing the body's natural processes to clear the infection.

This combination is significant in the treatment of lymphatic filariasis (elephantiasis). It acts as a "gold standard" therapy by effectively killing the young, immature worms (microfilariae) circulating in the bloodstream, thereby preventing the spread of the disease. • If you are being treated for a family-wide infestation (like pinworms or scabies), all household members should be treated at the same time.

• For intestinal worms, your doctor may suggest a repeat dose in 2 weeks to kill any worms that hatched from eggs after the first treatment.

• If you experience a mild fever or rash a day after treatment, it is often a sign that the medication is working and the parasites are dying.

• Avoid taking this medication if you have a history of seizures or neurological disorders without consulting a specialist.

• Ensure you drink plenty of fluids to stay hydrated if you experience diarrhoea as a side effect.

• Grapefruit Juice (CAUTION): Grapefruit juice may increase drug levels by affecting metabolism. Clinical impact is not fully established. Avoid or limit use unless advised.

• Neurocysticercosis / Seizure Risk (SEVERE): Treatment may cause inflammation in the brain as parasites are killed. This can lead to seizures or increased intracranial pressure. Use under close medical supervision.

• Bone Marrow Suppression (SEVERE): Use may reduce white blood cell counts. Rare cases of serious blood disorders have been reported. Blood count monitoring may be required during prolonged therapy.

An overdose may cause severe dizziness, vomiting, and loss of coordination. If an overdose is suspected, seek emergency medical help or contact a poison control centre immediately.

If you miss the scheduled dose, take it as soon as you remember. If it has been more than 24 hours since the missed dose, consult your doctor for instructions. Do not take a double dose.
